Configure a new supervised visitation tab (next to general, case file) for client info report information to be entered.
Take the information provided in the document attached and create a separate tab specifically for client visit info. We would need the ability to add more children info boxes since each case varies with the number of children per case.

This shouldn’t require any programming or system changes. You can ask John to setup a Report (either a Word Document or Custom Form) that contains this information.
