DFPS Service Plan is a nightmare we need a template that allows the custom form data to be printed on the DFPS form. This would help all CPA's greatly.
Please make this a priority

The solution here is to create the form as a Word Document or Web Document, so that it can have it look just like the DFPS form. You can use field codes to populate some fields.

I have the DFPS Service Plan form in Word format set as the template instead of using a Custom Form. I have it set-up to prefill using field codes, and it works pretty well for me.
