Electronic signature are not added when approved

Approval signatures will fail to populate a document, due to three reasons.

1.  The work's Signature is not store in their extendedReach Profile
2.  The document template does not have field codes for the signature
3.  The supervisor field codes are deleted by a worker, prior to the documents approval
